Vial
of Life
is a self-contained, emergency medical information kit created for any
member of United Cooperative Services or resident living with United’s
service area, including our elderly or disabled members, single parents,
teenagers or anyone with special medical needs.
The
program was specifically designed to provide necessary medical
information to emergency response personnel when a patient is unable to
do so. By minimizing vital seconds needed during medical emergencies,
“Vial of Life” can protect co-op members and residents of communities
within United’s service area and save lives.
The
Vial of Life kit contains an instruction sheet, two plastic pill
bottles, two window decals, two medical information sheets and a wallet
size information card. The medical information sheets are placed inside
each of the two pill bottles and placed on the top shelf of the
refrigerator and inside your vehicle’s glove box.
Decals
are placed on the front door or window at the main entrance to your home
and on the driver’s side windshield of your vehicle to alert emergency
response personnel to pertinent medical information stored in these
accessible locations.
The
wallet size information card provides important medical information when
away from your home or vehicle and also provides emergency personnel
with essential family and physician contact information for the
patient.
Vial
of Life
kits are available as a community service to residents within United's
14-county service territory and kits can be obtained from any of
United's seven local offices. The program is made possible through
funds from United’s Operation Round Up program.
